Case Management
The MS Society of Tasmania offers comprehensive case management services. These services can be helpful if a client is experiencing changes in their condition and require external support to meet their daily needs.
Case management involves MS staff working with the client, and possibly their family/carer, GP and neurologist and other service providers to develop and facilitate a plan of care to meet their individual needs, choices and circumstances. It often involves making referrals to other services and maintaining ongoing contact with the client and service providers to ensure that the plan of care is meeting the person’s identified needs and wishes.
